Many women arrive in my office during perimenopause feeling frustrated, confused, and sometimes betrayed by their own bodies. They tell me that they are doing everything they have always done. They are eating well, exercising regularly, taking care of their families, showing up for work, and checking all the boxes they have been told should lead to good health. Yet despite their best efforts, something has shifted. Sleep is no longer restorative. Weight seems to accumulate around the middle without explanation. Energy becomes less reliable. Anxiety appears in someone who never considered herself anxious. Brain fog makes simple tasks feel harder than they should.
It is easy to assume that these symptoms are simply the result of changing hormones. While hormones certainly play an important role, I have found over the years that perimenopause often serves as something much bigger than a hormonal transition. It becomes a window into the deeper story of a person's health. The body begins asking questions that perhaps it has been asking quietly for years, but now those questions can no longer be ignored.
One of the most important lessons I have learned as a physician is that symptoms are rarely random. The body is constantly communicating with us. Symptoms are often messages rather than malfunctions. They are invitations to become curious about what is happening beneath the surface. This is particularly true during midlife, when the physiological changes of perimenopause can amplify patterns that have existed for decades. The stress we have carried, the losses we have endured, the trauma we have survived, the sleep we sacrificed, and the emotional burdens we pushed aside often become part of the conversation.

This is why I believe that true healing requires a trauma-informed approach. Trauma-informed care is not about dwelling in the past or assigning blame. It is about understanding that life experiences shape biology. Every person sitting in an exam room has a story. That story includes not only diagnoses and medications but also heartbreak, disappointment, resilience, grief, triumph, relationships, and adversity. The nervous system remembers these experiences, and over time they influence how the body responds to stress, regulates inflammation, balances hormones, and manages energy.
Many people hear the word trauma and immediately think of catastrophic events. Certainly, trauma can include experiences such as abuse, violence, accidents, or major losses. However, trauma can also arise from chronic stress, emotional neglect, caregiving burdens, difficult relationships, medical procedures, financial instability, or growing up in an environment where safety and support were inconsistent. Trauma is not defined solely by what happened; it is also shaped by how our nervous system experienced those events and whether we had the resources to process them.
As we move through life, our bodies develop adaptations designed to help us survive. These adaptations are remarkably intelligent. The nervous system learns to stay alert in environments that feel unsafe. Hormonal systems adjust to chronic demands. Metabolism shifts to conserve energy during prolonged stress. These responses are protective in the short term. The challenge is that what helps us survive one season of life can sometimes become the very thing that prevents us from thriving in the next.

This is where the concept of metabolic flexibility becomes incredibly important. Metabolic flexibility refers to the body's ability to efficiently adapt to changing energy demands. A metabolically flexible person can transition smoothly between using carbohydrates and fat for fuel. Energy remains more stable, blood sugar is better regulated, and the body responds effectively to periods of activity, rest, nourishment, and fasting. In many ways, metabolic flexibility is a reflection of physiological resilience.
Unfortunately, chronic stress and unresolved trauma can interfere with this adaptability. When the nervous system perceives ongoing threat, whether physical or emotional, the body prioritizes survival. Cortisol patterns become disrupted. Insulin sensitivity may decline. Inflammation can increase. Sleep often suffers. Over time, the body becomes less adaptable and more reactive. What many people interpret as a lack of willpower or a failure of discipline is often a sign that the body has been operating in survival mode for far too long.
This is one of the reasons perimenopause can feel so disruptive. The hormonal fluctuations of this season often expose vulnerabilities that have been quietly developing for years. Women frequently tell me that they feel as though everything suddenly fell apart. In reality, the body may be revealing areas that have needed attention all along. Perimenopause is not the problem. It is often the messenger.
Rather than viewing this stage of life as a decline, I encourage patients to see it as an opportunity for recalibration. The body is asking us to pay attention. It is asking us to move beyond symptom management and toward a deeper understanding of what health truly means. This process takes time. It requires curiosity. It requires patience. Most importantly, it requires a willingness to look at the whole person rather than focusing on a single symptom or laboratory value.

This is where genomics can become a valuable tool. Our genes provide insight into how our bodies process hormones, manage inflammation, detoxify environmental exposures, regulate neurotransmitters, and respond to stress. Genetics do not determine our destiny, but they can offer important clues about our unique biological tendencies. Understanding those tendencies allows us to develop more personalized strategies for prevention, healing, and long-term wellness.
Genomic information is most powerful when combined with thoughtful testing. In my practice, I am interested not only in helping patients feel better today but also in understanding their future health risks. This means looking carefully at cardiovascular health, metabolic markers, inflammatory patterns, hormone balance, and bone health. Heart disease remains the leading cause of death among women, yet many women spend far more time worrying about the number on the scale than they do about their cardiovascular risk profile. Likewise, bone loss often develops silently over many years before it becomes apparent. If we are truly committed to healthy aging, we must broaden our focus beyond symptom relief and begin looking at the larger picture of lifelong vitality.
